Client Service Hours

The Pathology Center's Client Services Department is the day-to-day liaison for laboratory users. Physicians, hospitals and medical providers may call Client Services to:

  • Obtain test results.
  • Obtain specimen requirements.
  • Add-on tests.
  • Order laboratory supplies.
  • Refer you to the appropriate department personnel.

The Pathology Center is a participating provider for private insurance/health plans, Medicare and Medicaid.
When The Pathology Center bills the patient's insurance, include the following patient information with the requisition:

  • Patient's full name, date of birth and social security number (if available).
  • Patient's current address and phone number.
  • Please provide a legible copy of both sides of the insurance card(s) with the patient's insurance details, including company name, ID, group number, address, and policy holder name.
  • Appropriate ICD-10-CM code.
  • Advance beneficiary notice signature (if necessary).

  • ABN Medicare and Non-Medicare Waivers - Spanish
Testing Supplies
Use our online order form to request new supplies. Federal regulation requires that we only supply those supplies directly related to the testing performed at the Pathology Center.
